Annual License Usage Report Form

In accordance with the terms of your WLP Annual Deluxe Reprint License or WLP Annual Reprint License, and in order to help us to assure proper royalty payments to our composers and authors, please report your usage on a regular basis. You may use the online submission form below (simply complete the necessary fields and click "Submit") or you may download a PDF of the form and return it via fax, e-mail, or mail to the attention or the Permissions Department:

Please report on a weekly or monthly basis, and report all usage for the entire calendar year prior to December 15th.
